When an auditor asks “show me how this was handled”, the evidence pack answers in one download.

What the pack contains

For a single finding, the pack assembles:

• Its triage history — who looked at it and what they decided.
• Any risk exception and who approved it.
• The remediation requests, their approvals and results.
• The audit-chain entries that verify the fix, linked into the hash-chained audit trail.

Why it is tamper-evident

Because the verifying entries are part of the audit chain, the pack is tamper-evident: any later change to the history would break the chain and be detectable. The pack is evidence, not just a summary.
